Dear Student or Parent,
Concordia University is pleased to offer a new Student Insurance Plan, effective 8/1/2011, underwritten by Monumental Life Insurance Company.
All Full-Time undergraduate students and Athletes are required to purchase the Basic Injury and Sickness Insurance Plan unless proof of comparable coverage is furnished and waiver is completed by September 15, 2011.
Graduate students taking 6 or more credit hours (including online) are eligible to purchase this plan on a voluntary basis. Dependents of insured students are eligible to purchase this plan on a voluntary basis for the same period as the student.

- Access to the First Health Preferred Provider network for reduced out of pocket expense
- Increased the plan maximum from $25,000 to $50,000 Per Injury or Sickness
- Plan Deductible of $50 is waived for Outpatient Physician Visits
- Includes Preventative Care Benefits
- Includes coverage for Intercollegiate Sports up to $3,000 per Injury
- Accidental Death and Dismemberment Benefits

Enrollment / Waiver Options
Please review the plan Brochure carefully before making a decision on your health care needs. Students must either enroll in, or waive out, of the Student Insurance Plan by the deadline or you will be contacted by the Student Health Center.
- Students entering in the Fall, the deadline is 8/15/2011 to enroll or waiver coverage. Waivers received for the Fall semester will apply for the full academic year.
- Students who enter in the Spring, the deadline is 1/15/2012 to enroll or waiver coverage.
