Dear Student or Parent,
All full-time students are covered by the Basic Accident Benefit of $5,000 per Injury at no charge; this coverage is in effect for the 9-month academic year, from August 13, 2011 to May 18, 2012.
All full-time students are automatically enrolled in the Hard Waiver Accident & Sickness Plan and can only waive out the coverage by providing evidence of comparable coverage. The Hard Waiver Plan expands the accident and sickness benefits to 12-months, from August 13, 2011 to August 12, 2012, and the cost of the Hard Waiver Plan is billed to the students account.
Domestic Students
$155 (expands coverage to $10,000 for each Injury or Sickness). Additional Major Medical coverage for Domestic students of up to $25,000 or $50,000 for each Injury or Sickness is available by selecting Online Enrollment.
International Students
$575 (expands coverage to $50,000 for each Injury or Sickness)

If you do not want to submit your waiver on-line, you may download the waiver form; please submit the completed waiver form to the Bursar’s office. The deadline to waive out of the Hard Waiver Plan is August 12, 2011 for students enrolling in the fall semester and January 2, 2012 for those enrolling in the spring semester; these deadlines are strictly enforced and no refund will issued after these dates.
