Dear Student or Parent,
Out of concern for the health and welfare of all our students, Evert Tennis Academy strongly recommends that every student be covered by a comprehensive injury and sickness plan, one that meets the high cost of medical services and is accepted by local providers and practitioners.
Although many students are covered by private insurance, there are a few who lose coverage or are insured under an international or out-of-area managed care plan that may not provide coverage for your child when away from home. It is your responsibility to contact your insurance company to discuss what benefits will be lost and what deductibles and co-payments will be required when your child leaves home.
All boarding students are required to provide proof of adequate health insurance and complete the waiver form included in your admissions packet or Online Waiver.
For students who do not have adequate insurance coverage, we offer a Student Health and Accident Insurance Plan, designed especially for private secondary schools, underwritten by UnitedHealthcare Student Resources.

You may elect one of the following enrollment periods:
- 12 months (8/15/11 through 8/14/12) – $1,410
- Mid-term (1/01/12 through 8/14/12 – $ 885
- 10 months (8/15/11 through 6/15/12) – $1,280
- 2 Months (6/15/12 through 8/15/12) – $ 266
The effective date for enrollment (with payment) received on or prior to August 15, 2011, will be August 15, 2011. Enrollments received after August 15th, your coverage will take effect on the date your application and premium is received by the company. To enroll click on the Online Enrollment link.
