Dear Student or Parent,

Out of concern for the health and welfare of all our students, Sample University strongly recommends that every student be covered by a comprehensive injury and sickness plan, one that meets the high cost of medical services and is accepted by local providers and practitioners.

Although many students are covered by private insurance, there are a few who lose coverage or are insured under an international or out-of-state managed care plan that may not provide coverage when away from home. All full time students are required to provide proof of adequate health insurance and must complete the online waiver by the deadline of September 1, 2011 to remove the fall semester insurance premium from your student account.

For all other students who do not have adequate insurance coverage, we offer a Student Health and Accident Insurance Plan, designed especially for Sample University, underwritten by Carrier.

How do I enroll or waive out of the student insurance?

To review the plan benefits click on Student Insurance Brochure. To waive out of this plan, please complete the Online Waiver Form before September 1, 2011 or you will automatically be billed for the insurance and the cost for the fall semester will be added to your student account.

Things to Consider Before You Waive

  • Do you have coverage in Florida, including non-emergency treatment?
  • Access to healthcare providers with out-of-network benefits?
  • Will it cover a Pre-existing Condition?
  • Coverage for Hospitalization?
  • Reimburse for services provided by the on-campus SHS?
  • Pharmacy benefits?
  • Worldwide coverage?
  • Mental Health Benefits?

If you answered “No” to any of these questions, we recommend that you enroll either in the Basic or Comprehensive Plan.
